The Harrah Athletic Department would like to thank you for taking time to fill out the required athletic participation packet online.

Athletic Participation Directions:

1. Physical forms will be available at scheduled physicals. If your athlete is unable to attend one of these events you can click on the tab located at the top titled “Downloads”. The physical form can be printed out and taken with the student to their doctor’s appointment to receive the physician’s signature. Once completed these forms must be delivered back to Harrah administration.

2. To access the required athletic participation forms for Harrah Schools, click on the electronic forms tab “Electronic Participation Forms”. You will get a drop down list of all required forms.

3. To BEGIN, click on the form name and fill out the information requested. If you do not know your student ID#, NOT REQUIRED, leave the box blank. If a question is not applicable, please answer question with NA or UNKNOWN.

4. TO SIGN THE DOCUMENT, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using an iPad, you may create an electronic signature by holding your finger to the screen and signing or by using a stylus pen and sign.

5. You will receive a confirmation email once the document(s) have been reviewed by the Harrah Athletic Administrator
